Enrollment forms and documents
Licensed centers must have certain forms and documents for each child. In DropIn you fill them in once, and every DropIn center you book can use them.
The 7 items for each child
5 forms
- Child Information: your child's details, allergies, diet and medical needs, your details, the emergency contact and authorized pickup person, and the doctor
- Medical Authorization: consent for emergency treatment, doctor and insurance
- Developmental History
- Photo & Transport Authorization
- Topical Application Permission (for example sunscreen)
2 documents
- Birth Certificate
- Immunization Records
You can book without these. But the center cannot check in your child until all 7 are done.
Fill in the forms
Open Account > My Children, tap your child, then tap Enrollment Forms. Choose one of these:
- Fill in Forms by Voice: a quick spoken chat with our voice assistant. You review and sign each part as you go. The app needs microphone access.
- Fill in Forms Manually: type your answers.
At the end of each form, tick the box, sign with your finger, and tap Submit Signed Form. We email you a PDF copy. Tap View signed PDF to see it in the app.
Forms expire after 1 year
A signed form is valid for 1 year. The app shows Expires soon 30 days before, and Expired, re-sign after. Open the form and tap Update information to check your answers and sign again.
Upload documents
On your child's page, open Documents, then tap Birth Certificate or Immunization Records.
- Take a photo, or choose a photo or PDF file.
- Tap Add Page if the document has more than one page.
- Use Replace or Delete to change a document.
Add or edit a child
- To add a child, tap + at the top of the screen, or go to Account > My Children > Add Child.
- To edit a child, tap the child, change the details, and tap Save Changes.
